We offer a comforting Palliative Care
We are proud to work with the NHS Continuing Healthcare to care for individuals who are approaching the end of life, and who have decided to spend their final moments in the comfort and familiar surroundings of their own home.
Our palliative care team provides the support towards the end of an individual’s life, allowing them to pass in the most peaceful and comfortable way and in accordance with their wishes whilst promoting autonomy.
We work alongside District Nurses and other healthcare professionals to provide emotional support to the individuals and their families.
Our team’s duties and responsibilities include assisting with oral medication. We also assist with :
Personal care:
- Washing, showering and bathing
- Oral Care
- Toileting
- Dressing and undressing
- Pressure Area Care
- Repositioning
- Catheter Care
Specialist care:
- Stoma Care
- NG Tube
- Oxygen
- Peg Feed
Partnerships With Other Agencies
As well as recruiting a professional and dedicated team for our palliative care services, we work in partnership with health and social care professionals to ensure everyone receives the most appropriate medical and personal care from our experienced palliative care assistants, NHS Case Managers, GPs, District Nurses and Occupational Therapists. We have an excellent relationship with NHS Continuing Care.
